Does your coaches' age effect his effectiveness and should they be replaced at some stage? I have one nearing his 100th birthday soon.

Age doesn't matter. Just keep him if he is good.

If the game still exists after 10 years from now there'll be a handfull coaches well over 150 years old :-p

Does the height of the defenders counts in this game?

Height

Taller players will have better chances to win aerial duels when heading the ball. However, shorter players will be quicker in speeding up from a standing start (maximum speed will still be the same for players of all heights and is dependent on actual pace skill).

Does Form impact a players tactical discipline? ? I know it effects everything else.. thoughts?

No... tactical discipline is match experience + tactics experience, meaning the number of matches played with a particular squad and tactic... For example:
Total matches played: 100 league matches + 30 NT matches = Divine match experience
0 matches with tactic x = tragic tactics experience
Together = 0 + 17 out of 34 = 50% = Very Good tactical discipline
That's how it works basically...
