it is a bug - it's multiple injury during the match
when player get injured during, e.g. running with the ball or doing something which does not stop the game, then the game continues until foul, throw-in, goal or corner is given (or something else which stops the game) and during that time player may get another injury (one or even more) or he can get e.g. 10 slight injuries during the whole match (slight injury doesn't make him leave the pitch)
Raul said a while ago that they will fix this long lasting problem (a loooong while ago to be exact, might be even a year) - so far nothing was done as far as I know
