Team missing are for sure NT-team and U21 NT-team which are counted on total teams number in a country.
But which is the third?

what makes You think they are counted? I am not saying they are not, but I don't think they are. They are in different rankings, rankings of national teams and I don't recall seeing any NT team in country ranking.

It looks like missing teams are new teams, teams introduced in the current season. But not every club, maybe only teams created at some specific time.

E.g. in France 2 teams are missing in the national ranking (22 leagues, each one has 12 teams = 264, but only 262 are visible in the ranking) and these 2 teams are not both NT teams, but some normal teams from some leagues.
