I have had a few players who sat at 0-4 form for a LONG time, one is now divine (went up like a rocket second half of this season) and the other was around adequate when he got hurt.
I haven't seen any indication that playing or not playing affects form really.
Edit: Oops i haven't looked at the last two weeks for the player that was hurt. He went +6 the last two weks and is now incredible. Yes that's right, +6 WHILE INJURED.

My CD sat in tragic form for more than a season. A few weeks ago I started playing him twice a week, now his form is adequate.
its just like form in hockey arena. completely unpredictable
naaaaaaa, he is a selfish player. I should fire his arse.
I have not seen any pattern to the form. Just like everybody else, I might have a guy stay divine for 2 seasons and then suddenly tank. And then I might have a guy at tragic suddenly jump.
There's a rumor (started by greg lol) that greg doesn't like the implementation of form that much either, and that some day it may make it to his list of things he'll get to improve. It's just not his highest priority.

I have a player, Greg Fillmore, who has rarely popped above the average-adequate range.
A couple weeks ago, he was seriously injured, and his form dropped to hopeless. Since then, his form has actually IMPROVED every single week, 2-3 levels each time. Now he's sitting at excellent, and hasn't played in 4 weeks or so. It's raised his value about $60k since his injury. He has never been above $200k, and he's at $240k right now. Seems like somebody wants a pay raise next season =p

My unearthly+2 junior pull from the end of the last season have interesting form. He came out with very good form(like all juniors) and got injured for a month on his very first match. His form always raised and he got his form to divine when he was still injured. After healing his form stayed at divine for 8 weeks ! He currently has brilliant form, but I'm sure it will be back in divine next week...

I do feel sometimes that form follows the "need to rest" and "need to play" scheme.
I think form changes somehow depends on what kind of skills player have and where do you put him on the field. Some defs "like" to play deeper than others, and some mids "like" to play more inner than winger position etc.
But this is not the only thing what makes player form drop or rise.

Magnum, interesting theory. I have a guy who is a well-skilled central IM. But his form is usually tragic or hopeless, for two weeks he got to average. Skill-wise he is by far my best IM, but his form makes me wonder about his future.
