1) When a player has the same level of striker and playmaker, is it better to play him as an attacker or midfielder?

2) What levels of playmaker should a striker have as a minimum?

It all depends on what his other stats are like. If he has decent passing and some defending skill then he would be better as a mid imo, but better pace and tech would make him a better striker.

Passing is more important for mids than playmaking, but a striker with a decent level of PM makes decisions quicker, therefore he is more likely to pick the right option when in front of goal.

Playmaking is a slightly useful secondary skill on a striker - the minimum is tragic
but
Playmaking is an essential secondary skill on a Mid - but the Primary is Passing
or maybe they are joint primarys but no passing means he isn't a mid
